Filling out a Confidentiality Agreement is very easy.  Just follow these easy steps:

  1. Print this page
  2. Click on this line to display the Confidentiality Agreement, you will need to use the browser's "back" feature, to get back to this page.
  3. Using your browser, print the Confidentiality Agreement
  4. Fill out the blank spaces.(these spaces will print with a ____) for you to fill out.  To make it easy to find, there will appear red on the monitor screen.  You can scroll up and down to find them, however, you will not be able to input information using your keyboard. 
  5. The first blank is on the first line, it starts with "effective the".   Here enter today's date: (month,day,1998)
  6. The second blank is for your name or company name, and its type of business.  You can use my information as an example, "Mikeco.com, a California Partnership"
  7. The next block of blanks are found under 11. Notices. Fill in your name or company name and address. 
  8. At the very end of this document you must fill in with your name or company name.
  9. On the "By:_____" line, we need your signature.
  10. Below this, please print your name.
  11. Print your title.
  12. Lastly mail the completed agreement to:

Once we receive the document, and we agree to the terms, the document will be return to you with my signature.  Once the document has both of our signatures, the account information to access V.A.L.E.R.'s web site will be given.
