  8. At the very end of this document you must fill in with your name or company name.
  9. On the "By:_____" line, we need your signature.
  10. Below this, please print your name.
  11. Print your title.
  12. Lastly mail the completed agreement to:

Once we receive the document, and we agree to the terms, the document will be return to you with my signature.  Once the document has both of our signatures, the account information to access Prototype Service's web site will be given.
